Dr. Foldy practices and teaches family and preventive medicine at the Medical College of Wisconsin and consults in medical and public health informatics. He serves as Medical Director of My Choice Family Care, authors the health e.volution blog at sethfoldy.com and was interim CMO at Elli Health, Inc. (now Intellivisit) in 2014-15.
Dr. Foldy served as Distinguished Consultant at the US Centers for Disease Control and Prevention (CDC) from 2010-12 supporting public health in the transition to electronic health records and the adoption of national information exchange standards. He has also served as Health Commissioner in Milwaukee, WI and State Health Officer and Administrator of the Wisconsin Division of Public Health. He was co-founder and CMO of the Wisconsin Health Information Exchange, and has been medical director of outpatient operations at Cleveland’s MetroHealth Medical System and at Milwaukee Healthcare for the Homeless. Dr. Foldy is board-certified in both Family and Preventive Medicine, and holds appointments at the Medical College of Wisconsin, the University of Wisconsin at Milwaukee and the University of Wisconsin School of Medicine and Public Health. He received the 2002 Roemer Prize for Creative Local Public Health from the American Public Health Association among other honors.
